Multiple Sclerosis: Proper Diagnosis and Acute Phase Management

Abstract

Multiple sclerosis (MS) is an immune-mediated demyelinating disease of central nervous system. New evidence and suggestion has made the 2005 McDonald criteria for diagnosis of MS to be revised. The 2010 diagnostic criteria for MS simplify the process of diagnosis, preserve the diagnostic sensitivity and specificity, address the applicability across populations and allow earlier diagnosis and more uniform and widespread use. For the acute treatment of relapses, high-dose methylprednisolone is the routine therapy, but other treatments are also in current use. Short-term, high-dose methylprednisolone treatment should be considered for the treatment of relapses of MS (level A recommendation). Plasma exchange is probably efficacious in a subgroup of patients with severe relapses not responding to methylprednisolone therapy, and should be considered in this patient subgroup (level B recommendation). There is a need for further randomized, controlled trials in order to establish the optimal treatment regimen for relapses of MS.
